I most humbly and respectfully approach your honour with a request that I had admitted in BAI JERBAI WADIA HOSPITAL FOR CHILDREN, PAREL, MUMBAI under the treatment of Dr. PRASHANT HIWARKAR, emergency SEVERE COMBINED IMMUNO DEFICIENCY (SCID), and doctor have prescribed for Stem Cell Transplantation and Blood Products etc. advise me for expenses of Rs. 25,00,000/- (Twenty Five Lakhs) as I am age of 1.5 years and very poor and nobody is to help us as my father is working as a labourer on daily wages basis, and entire family depend upon him.
