I most humbly and respectfully approach your honour with a request that I had admitted in TATA MEMORIAL CENTRE, Parel, Mumbai, under the treatment of Dr. PRASHANT C. BHAT, emergency Acute Lymphoblastic Leukemia (Type of Cancer), and the Doctors have advised Haploidentical Stem Shell Transplant + Chemotherapy + Supportive Care, Chemotherapy etc. and advise for the period of 6 Months to me with expenses of (Thirty Lakhs ) Rs. 30,00,000/- as I am age of 5 Years year and very poor and nobody is to help us and my father cannot afford the expenses of hospital, as he is working as a labour on small skill basis and our entire family depend upon him.
